If you are registering a team as a logged in member of the HEADS SportsEngine network, please select your profile on the following screen entitled "Who Are You Registering" else, if you are not logged in you can continue as a Guest.
Once completed you may edit your entry at any time.
This form may initially be completed without registering any staff, however this will be required to be updated after initial completion and before the deadline. Information required should be ready when registering staff:
The deadline to complete this entry form is September 30th. All entries received after this date will not qualify the team for entry so it is critical that this intent form be completed prior to that date.
